Nadia,

I think that it should be possible to extend your J1 as long as you need for
a Ph.D.
I also think it should be possible that you can work with a J1 with the 2
year home residency requirement in a US company for approximately 1-2 years
after you have finished your Ph.D. This will be seen as job training.
When you plan to change your J1 to an other visa you need to have your 2
year home residency waived. Despite the fact that this should not be a
problem I know of people where it was not waived even their home country had
given the ok.
In a case like that you have maybe 100 days after your job training status
has expired until you need to leave the US or you will not be able to return
for a long time.

I think you have quite a few options and your school's international office
will be able to help you.
