Around 4 months in California, which is about the worst place in that
respect. YMMV though, it can actually take from under two months to over
six months.

Not if you don't already have an H-1B with another company (the law
changed recently to allow for that option which makes it easier to
change jobs).

Nope. But you can come to the US first, and then file for the H-1B while
you're in the US. Then you don't even need to get out of the country
until the H-1B is approved. You have to make sure that you're filing as
being already in the US though. I would consult with your immigration
attorney to confirm all this of course.
